[Bugs] MathML conversion doesn't handle + sign

Mostly in the title.

The case of the + sign being removed is interesting, considering that the - sign and other operators correctly show up.

Note that the operator does not appear in the output xml at all.

Hmm. Yes.

Thats easily fixed :) Basically '+' symbols seem to be converted into spaces by many browsers. So we just have to convert '+' into something else first, we usually use %plus;
